APPLICANT REQUESTS THAT:
His DD Form 214, Certificate of Release or Discharge from Active
Duty, be amended to show he was awarded the Military Outstanding
Volunteer Service Medal (MOVSM).
APPLICANT CONTENDS THAT:
He received the MOVSM after his review of his DD Form 214. He
thought his DD Form 214 would be automatically updated to
reflect the award but learned later that he would have to submit
his request to the Board.

STATEMENT OF FACTS:
On 1 Feb 13, the applicant was retired. He was credited with
28 years, 4 months and 2 days of active duty service.
The MOVSM, authorized by Executive Order 12830 on 9 Jan 93, may
be awarded to members of the U.S. Armed Forces and Reserve
components, who subsequent to 31 Dec 92 perform outstanding
volunteer community service of a sustained, direct and
consequential nature.
AIR FORCE EVALUATION:
AFPC/DPSID recommends denial. After a thorough review of the
applicants official military personnel record, DPSID was unable
to verify award of the MOVSM. Although the applicant provided a
copy of his Virtual Military Personnel Flight (vMPF) printout
showing the award, a source document could not be located
verifying the award. The source documentation for verification
of the MOVSM is a signed approval memorandum from the delegated
approval authority. To grant relief would be contrary to the
criteria established by DODM 1348.33, Manual of Military
Decorations and Awards, the Secretary of the Air Force and Chief
of Staff.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
He contacted SAF/PA, the office that awarded him the MOVSM.
They only had an unsigned copy of the memorandum; however, they
provided a Memorandum for Record (MFR) that states the SAF/PA
Director had signed the memorandum awarding him the MOVSM. The
award authority has retired and he has been unable to contact
him.
He knows the award was processed as his vMPF awards and
decorations record was updated.

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by
existing law or regulations.
2. The application was timely filed.
3. Sufficient relevant evidence has been presented to
demonstrate the existence of an error or injustice. We note
DPSID recommends denial stating they were unable to locate a
source document verifying the award of the MOVSM. However, we
find the MFR from the SAF/PA Director of Staff dated 4 Jun 14,
stating the award authority had signed the memorandum awarding
the applicant the MOVSM and the 30 Jan 13 vMPF printout
sufficient to substantiate the applicants request for award of
the MOVSM. Therefore, we recommend the applicants records be
corrected as indicated below.
4. The applicants case is adequately documented and it has not
been shown that a personal appearance with or without counsel
will materially add to our understanding of the issue involved.
Therefore, the request for a hearing is not favorably considered.
THE BOARD RECOMMENDS THAT:
The pertinent military records of the Department of the Air
Force relating to the APPLICANT be corrected to show that the DD
Form 214, Certificate of Release or Discharge from Active Duty,
issued in conjunction with his 31 Jan 13 retirement be amended
in item 13, Decorations, Medals, Badges, Citations and Campaign
Ribbons Awarded or Authorized, to include the Military
Outstanding Volunteer Service Medal.
